Forms may be picked up at our facility or printed from the web for your convenience. Once the enrollment forms have been completed and you have all shot records together, please call us at 904-246-9588 to schedule a short interview (Meet and Greet) prior to your dog’s first day of play. This will give us a chance to meet each other and give us time to assess your pet’s personality. We will help you decide if doggie daycare and/or Boarding would be beneficial to your dog and make sure you are comfortable leaving your dog with us.

PHYSICAL REQUIREMENTS

A copy of your current vaccination records must be provided for each pet prior to admission to our facility. For dogs we require DHLP-CPV (Distemper) and Rabies annually (or 3yr) and Bordetella annually, though we strongly recommend Bordetella be administered every 6 months. All vaccinations must be administered at least 48 hours prior to any services provided. Dogs MUST be in good health with no contagious illnesses or currently on medication for an illness, no Diarrhea (upset stomachs) Vomiting, etc. (If your dog is experiencing either of those 2 symptoms, we will require them to be picked up); free from fleas, ticks and matts; and should be given a routine heart worm preventative. We do not allow dogs in that are heart worm positive. They must have a calm and quiet environment during thier treatment.

SPAY/NEUTER POLICY

All dogs over 11 months old must be spayed or neutered in order to participate in daycare and/or boarding. Any female under 11 months of age cannot be in heat during their stay, if that happens an emergency contact must be able to pick up as soon as possible.

TEMPERAMENT

Because dog daycare and boarding requires a friendly, happy atmosphere in order to be successful, it is important that your dog be comfortable playing and socializing with other dogs. For the safety of all pets and staff, we require daycare and boarding dogs to be friendly to other dogs and people, willing to share toys, comfortable allowing a new dog into their play area, and be energetic.

We do not accept dogs that attack or have attacked other dogs, we do not accept dogs that attack or have attacked people, and we do not accept dogs that are destructive (destroy or damage property).


In order to ensure the safety of staff and all dogs left in our care, we reserve the right to turn away any animal unsuitable for our environment.
